1. Name of the artist(s) and piece(s) you are interested in
2. Your name
3. Shipping address, city, state, zip code
4. Your day and evening phone numbers (you will be called
    for your payment info)
5. If you will be picking it up after the show closes (8/15)
    or it will be shipped.

You may purchase as many works as you like.
